Hang on,
How did you create this floppy ? Was cp used? Or did you use dd ?
The image file is an image of a floppy disc, not an usable file.
To create a usable floppy from an image, use :
dd if=pc164 of=/dev/fd0
What you are using should then work.
You will probably find that the floppy is now minix formatted.

If your image is named "pc164"
try boot fd0:pc164 root=/dev/fd0 load_ramdisk=1 prompt_ramdisk=1
Simply replace "vmlinux.gz" in OSLOADOPTIONS with the real image name.
It's a good idea to start with no OSLOADOPTIONS
and figure out the right way/name on the milo prompt.
Try MILO> ls -t ext2 fd0
or MILO> ls -t msdos fd0
