The problem is not the images, it is the hardware.  Try another SCSI
adapter, or be sure that your adapter is working correctly.  From my
expernece with that type of problem, the hardware was not found duing
autodetection, and the software that you need is not loaded off the boot
images.

I had the same problem with a network card, the kernel did not find it and
insmod was not loaded off the images.  The second I changed the network
card to a 3com 509, the machine was happy. 

I would try a Adaptech AHA 2940, they are rather low cost and they work
'ok' in my 21164sx.
