The videocard chip is an ATI 3D charger, part nº 113-40602-100 and the
commercial name is ATI 3D rage II ,

Another good question is what kind of video card do you have in the
system? it may be one that is supported by AlphaBIOS but not SRM. Did
you
previously have AlphaBIOS on the machine by any chance?
(I changed my PC164 from AlphaBIOS -> SRM and don't recall having the
screen blank like yours is)
