Are you following the instructions at:

I believe that this is the procedure I used to update my AlphaBIOS based LX
machines to SRM, but it has been a while. I believe the .ROM file is only
used to upgrade from one version of SRM to another.

Hello,
I am still out of luck to upgrade AlphaBIOS to SRM on my 164LX
motherboard - SRM prints a lot of debug info and
say machine check on vector 670. :( Linux works just fine.
I tried to low clock frequence to 433Mhz, but it didn't help. Here is
configuration I have:
motherboard - Digital 70-32811-C0 A01/54-25002-01 B03
CPU - Samsung made 533Mhz Alpha (works on 600 Mhz)
video - #9 (S3 Trio64)
scsi - NCR825
There are two versions of SRM image available for download (none works
for me):

ftp://ftp.digital.com/pub/DEC/Alpha/firmware/archive/pc164lx/lx164srm.rom
May be someone has previous versions of those files?
