Since I'm not an attorney, I can't really answer this.
I can only give my opinion as a consumer. I believe the copyright on the
web page design prevents someone else from using the same design elsewhere
and for other purposes. In my view, therefore, building a completely
different form that provides RA all of the data and sources the same
distribution channels would not be any kind of violation. I think of this
approach much like a translation into another language--which can be
achieved through proxy servers. The difference in this instance is that
the translation is to a medium more appropriate to a class of users,
rather than to a language group.

At the time we at AFB posted our form, we were in contact with RA and were
trying to get them to make their forms friendlier. They did make some
effort in this direction, but did not really listen to us, so their
efforts were only marginally better. Also, they took long enough that we
simply went ahead. After all, we had a conference scheduled, and the
Chairman of the FCC coming to talk about Sec. 255. We wanted people
everywhere to hear that.
