You might want to also address the MD(ADM) list (linux-raid vger kernel org) with this. I was seeing similar issues with raid10 prior to reporting several other issues, which are now fixed in 2.5.5 of mdadm. I haven't seen this error since.

Try replicating it after re-creating the array with mdadm-2.5.5, and showing the mdadm -D /dev/md{n} && mdadm /dev/md{n} -E /dev/$member_dev
