Yes, you are correct, you can create it on an unmounted fs. However,
it is NOT possible to create it on a read-only mounted fs, because
it needs to be able to write into the filesystem to create .journal.
To create a journal on root, you need to boot to another kernel (even
one that doesn't support ext3 will work), or boot from a rescue disk.
