mkinitrd is still broken on my system. I'm getting the mount error 6 on an ext3
system. Its trying to mount device 0803 which is off an adaptec controller. None
of the 4.0.x  mkinitrds have worked so far. I always have to backtrack to the
trusty old 3.x - which also causes me to downgrade initscripts back to 7.59.

Are there any troubleshooting steps to help find the problem?
