next time you see this happen, before you reach for a "clean all"
solution, see if "clean metadata" is enough to solve the problem. 
Depending on when the metadata cookie was generated in your cache and
your cache expiration settings, you can get into a situation where all
the mirrors have updated their metadata and your local metadata cache
is out of sync with all available mirrors. This is difficult to
reproduce and greatly depends on the timing of mirror syncing versus
your metadata cache creation and expiration.
