what's the recommended structure for an LVM install?

i just want to clarify the most common top-level structure for an LVM-based
install, since different docs seem to suggest slightly different layouts.
first, which partitions should be primaries *outside* LVM? i've chosen two
primary partitions -- swap and /boot -- with all else inside. but i've seen other
docs that suggest putting swap inside. and i've also seen docs recommending
keeping the root filesystem in a primary partition, although i''m suspecting this
might have been written by someone who didn't consider a separate /boot
partition.
what are folks commonly doing these days? thanks.
