do you know the device?
it's usualy /dev/sda because the reader is "like" scsi device.

Could you check the /etc/fstab file?
Could you try to mount manualy the device and read the error occurs?

#mount -t vfat /dev/sda /mnt/flash
or try 
#mount -t vfat /dev/sda1 /mnt/flash

if error occurs send mail with output
