I've done what you said, unfortunately nothing new :-(
Since you upgraded, the map probably still has references to /dev/hda
vs. /dev/sda as it was in my situation.
I've already modified wherever was necessary: /boot/grub/grub.conf,
/boot/grub/device.map, /boot/initrd[...], /etc/fstab.
I hope I didn't miss something else.
