[linux-lvm] lvmcreate_initrd don't find module lvm.o

That's not a problem if lvm is built into your kernel (it won't need
the module).
I found that my lvmcreate_initrd (w/ SuSE 7.0) was missing "mount",
"umount", and "/proc". Modify the script to include these, if necessary.
Note, also, when creating the initrd, the current system has to be
booted with 8MB ramdisks (either specified to lilo or in the kernel
build).
