Arie,

first of all: why are you using an older version of LVM1?
You should upgrade to 1.0.5.

Should not be needed, because a modprobe is inserted into /linuxrc by
lvmcreate_initrd. Strange.

This could be caused by missing /dev entries for the disks failing vgscan
to find some/any PVs.
Did lvmcreate_initrd show any errors?
