Actually, that brings up something interesting. If you switch the mouse from
the KVM to directly connected do you have to reboot or just restart X? If
simply restarting X fixes it, without requireing a kernel reboot, then it would
seem unlikely to be a kernel problem.
There should be a few lines in dmesg about the kernels mouse detection during
boot, those lines would be interesting as well.
I'm interested in this because I have a friend who is having a similar problem
with his KVM on his system at his home office. I've been trying to work with
him on it and your problem seems to be an exact match some I'm hoping to extend
the amount of data I'm collecting, and, if we get lucky, maybe find a fix.
