If you're ok with using the Qlogic failover capabilities, look at:
http://dag.wieers.com/home-made/qla-autoconf/
Currently it requires some knowledge about the Qlogic configuration (which
is explained with the documentation that comes with the driver) which is
not that easy.
I can assist you if necessary, in most cases I only need the Qlogic /proc
output and a little information about the backend topology and what you
require. qla-autoconf has a few examples
