rejecting I/O to dead device



I just upgrade a dell 1650 from RHEL3 to RHEL4.  The upgrade went
smoothly, however when I rebooted, the machine came up to a login
prompt, but then starting spitting out tons of errors.  I can no
longer log in as none of the commands are preset (it's like the hard
drive doesn't exists).  The errors I am getting are:

scsi0 (0:0): rejecting I/O to dead device
EXT3-fs error (device sda2): ext3_find_entry: reading directory #XXX offest
EXT3-fs error (device sda2): in start_transaction: Journal has aborted.
</snippet>

These errors keep occuring.  Is this a result of the upgrade or is
something more pressing wrong here?  I've tried booting in knoppix to
recover and it couldn't detect the CDROM drive or the hard drive. 
Also, I was able to boot into linux rescue using the RHEL4 CD's and I
was able to mount and chroot the drive fine.  So I'm now lost whether
it's an upgrade problem or a hardware issue.  Does anyone have an
similar experiences?
