Well, as you asked for the minimum of necessary packages for an
install as well, my first assumption would be that you missed one of
the vital ones. Unfortunately, I can't tell you which one is
containing mkinitrd, but I just found out that there is a new package
list at http://www.redhat.com/linux-info/pkglist/rhl-i386.html
