I had similar problems with my CD. My mirror wasn't preserving permissions
correctly, so the CD I burnt didn't have execute permissions. I added
execute permission to RedHat/instimage/usr/bin/*, wrote another CD, and
everything worked.

The second stage loader, I believe, is RedHat/instimage/usr/bin/install2,
which would be consistent with this.
