Hello Geoff,

                In the present company I'm embarassed  to admit that I know
the answer to at least part of your question. In  Win 95 or NT, and, I guess
Win 98 do:

    Start
        Settings
            control panel
                system
                    device manager
                        Radio button for view devices by type
                            Click on the + by the picture of a 'phone on a
box
                                Click on the modem name to highlight it
                                    Properties
                                        select the resourses tab


It will tell you the IRQ and I/O addresses.
