Are you using mgetty or vgetty in inittab?  I experienced this too
before switching to efax.  I never did get all of the quirks worked
out.  Though switching back to using the old netcfg instead of rp3 to
setup ppp fixed the double dial issue.  (rp3 has several other bugs that
made it unusable for me anyway...IP aliases make it blowup, even after
updating to the version that was supposed to fix it.)
