It seems to be a PC-Tel HSP56 MicroModem Audio Modem Riser (it plugs
vertically into a set of pins on the back of the mother board).

Looks like the modem is mentioned on the front page of linmodems.org - it's
a PCChips motherboard. I'll try the driver that's linked to from that page
and see if that works.

Thanks (again) for the pointer.... I'd never have found it!
