Is this the same CD that worked on another machine? If not, it might be
bad.

Try making a boot disk (install type) using an image file in /images
(unix) or \images (do$) off of the CD. It's made with the file rawrite
in /dosutils or \dosutils. The instructions on use are on the CD. That
makes a bootable install disk. But, if the CD is bad, it might still
give you problem depending on what is wrong with it.
