mount vs. fstab

Hi,
Can one of you knowledgeable people help my education a little bit?
Thanks in advance.
Q1) To mount a drive MUST there be an /etc/fstab entry? Or can I create a
mount point and mount by hand in any case?
Q2) If I must have a mount point, is any form or restart or reboot required
to make a change in that file seen?
Q3) With hot-plug media (SCSI, USB, 1394) do I need to do anything
differently? I'm concerned about remounting a device on the correct mount
point.
