Looking for a script

I'm not entirly sure that this is the right place to ask this, but I'll
try.

I'm looking for a script that I can place on one of our internal web
servers (a RH7.3 box ), so that people who come to our website....
(we're on an intranet), can add thier name and address, and email
address, for part of our internal recruitment campaign.

It would be nice of the data could be stored in a MySQL database, so at
least we can import it into our *cring* MS databases.

Any help would be appreciated. If I knew where to start, I'd probably
write a script myself, but I'm lazy, and pretending to be busy! lol !
