I've never tried this feature, but there are two things I can recommend:

1) the /etc/usertty file probably needs to have restrictive permissions;
like possibly 0400.

2) support for usertty might not be compiled into the binary you are using
(ie, the man page is wrong).  Try getting the source to login and find out
what it takes to enable it.
