The following table details how the board will be set with one or both of
these jumpers installed:

                I/O Range                 W2     W1
                ----------------          --     --
                200h - 21Fh               ON     ON
                280h - 29Fh               OFF    ON
                EISA mode                 ON     OFF
                Normal operation          OFF    OFF
 
 So, short answer is, remove the jumper blocks on the card marked w2 and w1
and you're probably in business. If you still need the setup.exe file you
can probably find the current version on the DEC (oops, COMPAQ) web site. 
If you want the really ancient one I have, I can zip the drivers disk and
put it up for ftp, or file attach it, or something......
