This was at root. And to answer Mike Burger's point, yeah, the floppy was 
not write-protected.

ran this:
mkbootdisk --device /dev/fd1 2.4.2-2
spat out a bunch of numbers, then:
floppy1: floppy timeout called
end_request: I/O error, device 02:01 (floppy), sector 0
mkdosfs: unable to open /dev/fd1
Failed to format /dev/fd1
