One other thing (besides FW settings).  If you installed ssh
   ./configure --with-pam
then make sure you have an entry in your 
    /etc/pam.d
directory.  That has nailed me more times than I can count.  If that's
the problem, the debug process I specified in my last message should
bring that out.
