I use a program called 'mailfilter' which is downloadable on freshmeat.
It deletes mail at the server before downloading.
It isn't perfect, and can take some work to get the filters right. I had
a heck of a time with Korean spam, but I haven't received any for a few
months now.
I set it up with a script called by cron that checks and deletes mail
and downloads the rest with fetchmail.
