The permissions on /dev/lpX (usually lp0) may be wrong (crw-rw---- on mine, owners root.lp). I've also had spool directories go bad. What I did was remove the printcap, stop lpd, "rm -rf /var/spool/lpd/*, recreate the printer and restart lpd. It's worked for me before. I'm pretty sure it's worked every time I've tried it, though I can't be absolutely sure of that part.
