Error 127 with MySQL on RedHat 7.2

A customer of my employer is continuously getting this error. They fix
their database files, and the error returns between a few hours and a
few days later. They want to send me down to work on our product, but I
cannot do this while this error exists. I have seen many causes, but
none seem to be the problem in this case.(examples a file over 2GB could
cause this, but they do not have this) 

They are running the following:

Redhat 7.2 Kernel 2.4.7 (SMP)
2G Ram
3 36Gig SCSI Raid 5 setup using an ext3 file system

MySQL 3.23.54

Any clues as to things to look into?
