I've looked through the logs, but given that the info on logical drive 2 is
all ok, is this the *current* info, or what was found immediately after the
failure?
Just to restate the obvious, this *is* h/w RAID control.
The more I look, and the more I research, the more it looks like a dirver or
hardware issue. For example, I found

where someone's having what *seems* to be a similar problem.
So, several thoughts:
- what did y'all have to do to rebuild the array?
- is the array terminated properly? (no chance of the terminator
being jostled out of full contact, is there?)
- have you run whatever tests you can on the ServeRaid controller itself?
