In my case, it did not work with USB2.0 card, but worked with USB1.1 card/port.
Try following steps:


Check no device is already mounted on /dev/sd*
Remove ehci-hcd module if it is loaded
Reinstall  usb-storage

I do not think the following step is necessary, but....
If your flash disk is still not found, run rescan-scsi-bus.sh [you can get it from the following site]
