Yes, it croaks immediately in %setup.  As soon as it inspects the file
it wants to untar.

OK, what about rpm -vv -V rpm ??  

Did you try re-installing rpm?

I've certainly seen lots of problems go by on 4.0.x, I don't pretend to 
follow them all.

But you seem to have done the right things upgrading your system the
(new) standard RH way and it should work for you.  

So it would be helpful to find out why it doesn't.
