Qualcomm's qpopper, used to be packged for redhat, I don't know if it 
still is.

When compiled in server mode and with support for a hashed mail spool, 
it is very
good. We have been using it since 1996 and have ~14,000 mail accounts. 
We use
a two character hash {/var/spool/mail/u/s/user} to keep the number of 
entries per
direrctory managable {~20 entries per directory for 14,000 accounts}. 
This also
lets you use multiple drives/partitions {average storage requirements 
are 1MB per
account, 14GB for 14,000 accounts}. We use 2 sets of RAID mirrored 18GB 
SCSI
LVD drives. We have good performance, and enough headroom to keep going 
even
during spam and virus storms.
