[patch] TUX 2.4.2-M2

i've uploaded -M2:

Changes since -L9:

- IO-cachemisses were not redirected to async IO threads in all cases.
  (this bug got introduced in -L6.)

- req->error got erroneously propagated over to the next keepalive request
  in certain cases involving user-space modules. (introduced in -L6.)

- cleanups to handling userspace-request error handling.

-M2 is looking pretty good - it passed various stresstests, and all the
corner-cases appear to be handled correctly again. This should be the last
patch for today. As always, bugreports, fixes, comments, suggestions
welcome.
