[dm-devel] [PATCH] coda: rename REQ_* to CODA_REQ_*



REQ_* constants are used for block layer requests causing inconsistent
duplicate definitions of REQ_WRITE.  Rename REQ_* used by coda to
CODA_REQ_*.
