Looks like you hit one of the two big ext3-0.0.6b-fixed bugs: the
original orphan code was reading the inode's orphan list pointers
before taking the superblock lock, so by the time it came to update
the pointers, the original list might have changed.

Bad boy, you shouldn't be running such an old version!  Upgrade
immediately!  This should be fixed in 6b.
