This is not an ext3 problem.  It's an IDE problem, and most probably
has been caused by a bad kernel config.  The IDE driver has had an IO
request when the filesystem tried to read the superblock (sector 2 on
the /dev/hda5 disk).  

Again, I'll ask: what does the IDE probing look like during boot?  The
full ".config" used would also help.
