Actually it's Linux (or GNU/Linux if you want) but not Unix.
Most maintained MUAs support Maildir:
- Evolution
- Kmail
- Balsa
- Mutt
- Mew
- Gnus
- Pine (with patch)
- Sylpheed (through maildir2sylpheed)
"Expecting" mbox is a matter of default configuration.
What MLM needs a mbox for incoming mail?
