Get rid of impossible XML call to an entity external to this infoset.
I've changed the rendering used in bookinfo.xsl to put the version
number of a document plainly on the front matter, so it's easier for
people to file bugs against the right version.  Note to self: what a
difference a year makes -- don't do dumb things like this anymore if
you can avoid it.
