You need both of these:

sysctl -w net.ipv4.tcp_default_win_scale=0
sysctl -w net.ipv4.tcp_moderate_rcvbuf=0

It took quite a bit of Googling to find those.  Without them,
traffic to my server from within my LAN as well as inbound
traffic to my server from the Internet was slow, but doing
FTP transfers from the Internet to my notebook on my LAN
worked fine.  The notebook runs WinXP, the server is my
Internet router.  Weird.
