Looks like it's BusLogic.ko to me.

Try appending "alias scsi_hostadapter BusLogic" to /etc/modprobe.conf.  If 
you already have a scsi_hostadapter alias defined, use "scsi_hostadapter1".

You should now be able to create a bootable initrd image with mkinitrd.
