Nope, include the dir and ghost the file. Then people might get a clue that
there is something inside, e.g.

%dir %{_localstatedir}/cache/afraid-dyndns
%ghost %{_localstatedir}/cache/afraid-dyndns/afraid

I wouldn't say that the separate dir is a must for you as upstream developer,
it's just a recommendation for packaging in Fedora similar to libexecdir:
"%{_libexecdir}. Packagers are highly encouraged to store libexecdir files in a
package-specific subdirectory of %{_libexecdir}, such as
%{_libexecdir}/%{name}."

This may also help with SELinux issues, I know this from bad experience. :(
