Do the machines where this happens have anything in common?
What arch? Multiple hard-disk drives?

Installing a new kernel pkg runs grubby, which in turn reinstalls
GRUB instead of simply modifying grub.conf. What could go wrong but
not always? It's also interesting that GRUB seems to lock up instead
of printing any of its stage-file-loading related error numbers.
