I had these issues on a crappy motherboard with a VIA chipset.  Since I replaced
that board I haven't had this problem, although I do get a kernel oops if I
unplug the antenna from the dvico while it is in use.

The original problem wasn't just that the usb system died, but that when it did
it brought down the ps/2 keyboard with it.  I can't reproduce this problem
anymore, but I hope someone takes the time to fix the kernel error handling.
