[Linux-cachefs] Ceph, outstanding request error

I'm been making slow progress ceph caching working. However, I'm stuck on
this error now:

FS-Cache: Cookie 'CEPH.inode' still has 1 outstanding reads


I see it in the log a few times. I'm sure I'm doing something incorrectly
and I would appreciate a pointer to some possible things I'm doing wrong.

I'm testing this against linus' master. I had some Oops with your fscache
branch so I came back to mainline.
