What version of the code?
Have there been node failures?

Someone is trying to use LVBs and doesn't have the lock[s] in the correct
state to do so. This shouldn't ever happen. (we have seen this before,
but I'm not remembering the conditions that caused it.)

With LVBs not working, gfs cannot cache any info there. (or the info is
wrong) This will affect df, quota, and posix range locks (on 5.2.* and
6.0.*) df and quota will be off, posix locks will probably panic.

You really should shutdown/restart everything as soon as you can. This
is the easiest/quickest way to fix it.
