Are the magma plugins installed? CCS uses magma to connect to the cluster manager - which can be either gulm or cman. If the plugins aren't there, it will not be able to contact the cluster manager and won't be able to determine quorum.

ls /usr/lib/magma/plugins

Try this on all your cluster nodes:
1. modprobe cman
2. ccsd
3. cman_tool join -c <cluster name>

before bring up any other subsystems, do
4. cat /proc/cluster/status

Total_votes should be >= Quorum
